Solutions provided:

Remove all insulation and air seal all penetrations from hvac, electrical and plumbing. Air seal all cracks and crevices with insulating foam. Install baffles at eaves to keep insulation in proper position. Disinfect attic and blow in all new cellulose insulation treated with insecticide and fire retardant. Insulate to a R49 level, about 13 inch depth of insulation. Weather-strip and insulate attic hatch.
